How do I initiate refills?

To initiate refills or dose changes please fill out "Weigh Day" form. You can access this form at any time on the patient portal. "Show Menu"--> Other forms--> Custom Questionnaire --> chose practitioner Melinda Curtin -- select questionnaire GHN CHECK IN Weigh day 2024 --> (MOBILE scroll right) there is a blue button to choose "Create form"

How do I receive messages ?

A notification will be sent to your email.​ You will then need to log into the Optimantra account by using the button above. On the left side on a phone there is a tab that says "show menu" there is a tab called messages.

Optimanta- the portal will send you text messages - these are automated and ONE way only.

Where will I pay?​
-
It is mandatory as agreed to in the consent that credit card information is saved. Depending on the medication you are prescribed you will either be automatically billed on the 1st of the month. If you have not yet saved your card information an invoice will be sent to your email. Prescriptions will not be sent until invoice is paid.​​

To request a prior authorization please fill out a form. Log into portal "Show Menu"---> Other forms--> Custom Questionnaire ---> chose practitioner Melinda Curtin --> select questionnaire GHN Prior Authorization request --->MUST SCROLL RIGHT on phone there is a blue button to choose "Create form" Once completed you will be sent a one way text message when complete.​​

What if I want to talk to you? How do I make a video or telephone appointment?

Go to Online booking --> existing patient --> choose telephone call, video call or "cold call" ( cold call will be at a random time during the day when I have time but will most likely be sooner than appointments are available).
