How do I initiate refills?

To initiate refills or dose changes please fill out "Weigh Day" form. You can access this form at any time on the patient portal. "Show Menu"--> Other forms--> Custom Questionnaire --> chose practitioner Melinda Curtin -- select questionnaire GHN CHECK IN Weigh day 2024 --> (MOBILE scroll right) there is a blue button to choose "Create form"

How do I receive messages ?

A notification will be sent to your email. You will then need to log into the Optimantra account by using the button above. On the left side on a phone there is a tab that says "show menu" there is a tab called messages.

Optimanta- the portal will send you text messages - these are automated and ONE way only.

What if I want to talk to you? How do I make a video or telephone appointment?

Go to -->Make appointment --> existing patient --> choose telephone call, video call or "cold call" ( cold call will be at a random time during the day when I have time but will most likely be sooner than appointments are available).
